okay so this has only recently been discovered how to unlock, i'm going to put it under spoilers just incase anyone would rather find it themselves.
there's a secret voice clip that can be triggered in the stairways of Chapter 3 - one of the stairwells has a set of instruments.
as you might guess, this voice clip belongs to Sammy.
you can hear it (and how it's triggered) in this video:

if you can't see the video, you play Cello, Drum, Violin, Piano, Drum in that order, and then you hear:
"We've all been waiting, but now... He will set... us... free..."
Sammy's voice isn't as clear as his "I said, can I get an amen?" line but it's definitely not a recording, it sounds echo-y like its coming from the walls or reverberating from another room.
apparently this voice clip has been in the files for a while, and even the code relating to it was found (which specifically mentions Sammy which was neat), but only yesterday was the combination figured out!
so what do you guys think? i think this tells us Sammy isn't dead. he sounds like he's... somewhere. i've always been dubious that he even died after Ch2 so... hmm!!!!!!!
